NPMA 99A Forms for Termite Protection must be completed by the builder and are submitted for proposed (new) construction cases when prevention of subterranean termite infestation is specified by the builder or required by the lender, the architect, FHA, or VA. NPMA 99A Forms are the Subterranean Termite Protection Builder’s Guarantee which is issued by the builder to the buyer as part of the construction process to prevent termite infestation.
This form is the latest, updated form which was released in July 2024. It comes with an official OMB Approval No. that expires 7/31/2027.
Please note that all previous versions of NPMA 99A forms are no longer valid for termite protection documentation.
NPMA 99A Forms are 4-part, all white paper carbonless forms and cannot be personalized with any pest control company’s information, as it is not required.
NPMA 99A is one of two HUD Pre-Treatment forms for any treatments required for new construction to ensure compliance with HUD guidelines. The two forms required:
- NPMA 99A Forms for Termite Protection (completed by builder) – commonly referred to as HUD Form 2580 NPMA 99A.
- NPMA 99B Termite Soil Treatment Record (completed by pest control company) – see product 2581 NPMA 99B for more details.
NPMA 99A Forms are used in conjunction with NPMA 99B Forms (when applicable), in accordance with Section 24 CFR 200.926d(b)(3), which requires that construction sites for HUD-insured structures must be free of termite hazards. The licensed pest control company will provide the builder a record of specific treatment information in cases when any method other than the use of treated lumber is applied to prevent subterranean termite infestation.
